RX Issue Lab Check

Team RIA

Please use this form to submit your prescription for inspection directly to our RX lab. Our lab techs will review your prescription information and ensure that everything was processed properly. If the lenses were processed incorrectly, we will reach back out to confirm that we will be redoing your lenses, free of charge. If the lenses were processed correctly, we will reach back out to help with a different resolution.
